REGISTRATION CHECKLIST 2017-18
For Parents of Students New to Kenrose

Welcome to Kenrose! The following information explains the requirements for completing
registration in Williamson County Schools. All items must be completed before enrollment can be
completed and your child/children can attend school.
Note: WCS enrollment forms, including the Enrollment Checklist 2017-18, are located on the
Williamson County website at http://www.wcs.edu/enrollment
Step 1- As soon as you know you will be moving to our school zone please contact our school office
(615-472-4630) to provide:
 Child’s name, grade, date of birth, and gender
 Parent names and contact information (email addresses, cell phones)

Step 2 – As soon as possible, complete the following enrollment forms, available on the Williamson County
Schools website http://www.wcs.edu/enrollment/ and bring them to our school office:
All Students - Enrollment Form 2017-18; Proof of Residency 2017-18; WCS Health History and Emergency
Contact Information 2017-18; Technology Acceptable Use Agreement for 2017-18; WCS Parent Access
Agreement 2017-18; McKinney Vento Questionnaire 2017-18; Migrant Occupation Survey 2017-18
Some Students as Needed - Request for Waiver of School Fees 2017-18; Out of Transportation Zone Request
for School for School Year 2017-18; Request to Return to Zoned School 2017-18
Step 3 - Also bring the following original documents to our school office to be copied:
_____ Proof of residency in the Kenrose school zone
o Signed lease agreement or Settlement Statement and
o 2 different utility bills or connection verification from the utility companies (WATER, GAS or
ELECTRIC ONLY)
_____ Certified Birth Certificate or Passport
_____ Custodial Parent’s Photo ID
_____ TN Dept. of Health Certificate of Immunization
_____ All foreign born children entering a Williamson County School for the first time are required to present
evidence of freedom from infectious tuberculosis. The evidence should include a report of a negative tuberculin
skin test done in the United States. If the skin test is positive, a chest x-ray or other necessary examinations to
certify absence of clinical tuberculosis is required.
_____ Proof of current (within 1 year of 1st day of school 8/10/17) Physical Examination
_____ Parenting Plan (if applicable)
Step 3-Complete these forms at our school office:
___ Records Request Form (Grades 1st thru 5th only)
(Records Request Form is to request records from your child’s previous school and is not
required for new Kindergarten Registration)
___ KES Student Placement Form (information to be used in placing your child in a class)
All of the above must be completed and confirmed by our attendance secretary Terri Tidwell
