Is this card a replacement card? Yes / No - If ‘yes’, a £5 fee is payable with this application.
What you need to do:
 Fully complete the information below. Please use your given name, not a short version.
 Write your name and DOB on the reverse of a passport photograph. (45mm x 35mm (1.77 inches x
1.38 inches). The photograph should be taken against a white background and must show your full
face. Other forms of image cannot be accepted as the quality is poor.
 Attach the passport photograph to the form.
 Return this form to your Institutions Higher Education office.

 Check all details and that the correct Programme and Plymouth University Code are entered.
 Approve your application and forward to Academic Partnerships at Plymouth University.
Please note: your card cannot be issued until your enrolment form has been forwarded to Plymouth
University Academic Partnerships office.
 Issue your card once it has been returned to the College Office.
What Academic Partnerships at Plymouth University will do:
 Confirm that your enrolment with Plymouth University is live.
 Request a card on your behalf.
 Return the card to the Institution Higher Education office for collection by you.

For information:
Your card is valid for the full period of your programme.
If you progress to a Plymouth University top up year the end date can be extended.
Your photograph will be stored electronically and will be accessible to staff of the
University. It will not be released to any external agency.
The Plymouth University card allows you to gain access and borrow from the Plymouth
campus library. The Plymouth University card allows you to use Plymouth campus
sports facilities (charges apply).
