You can find the concessionary bus pass download on your local government website; each area has its own
form.
This is the form and the procedure for the Leeds area:

You will need to click on application form (PDF 270KB) and print the form or alternatively you can use the hyper

Use the print button which should appear when you place your cursor over the top of the page on the right
hand side of the screen just below the address bar as highlighted in red below:
If any of the Part A automatic criteria apply to you, tick this and provide the proof stated on the form, send
photocopies and not originals of any proof asked for. You do not fill in Part B.
If Part A automatic criteria, doesn’t apply to you, you will need to fill in Part B discretionary criteria. Autism
spectrum condition and Asperger’s is classed as “I have a substantial learning disability” on this form.
Part C needs to be filled in with the details of the person who the bus pass is for.
Part D needs to be filled in if you have filled in Part B by your G.P. or any other medical professional who you
see about your condition. They will need to stamp in the validation box with their official stamp. You will need
to decide whether or not you require a companion pass to access the community with a support worker or a
family member so you can apply for a companion pass before they fill in Part D as they will also need to fill in
Part E section 2.
If you feel you require a companion pass to access the community with a support worker or a family member
and Part E section 1 applies to you please tick this and send the proof stated on the form.
Part F needs to be signed, dated and name printed by the person who the bus pass is for.
Please use the check list in Part F as you need to provide a passport sized photo which has been taken in the last
6 months with the applicants name printed clearly on the back.
If you have had Part D filled in by a medical professional or your G.P. you will not need to send any further proof
of your condition, you can always ask someone who supports you to fill in the additional information or
supporting evidence section at the end of Part G.
If you are applying for the first time you will need to send a photocopy of ONE confirmation of address and a
photocopy of ONE confirmation of identity listed on the form in Part G.

Your new pass will not be issued immediately and the government website for Leeds advises to allow three
