Service Reporting System (SRS) On-Line Information Sheet
Before you can begin this process, you must first contact the site where you will be serving and schedule your first day of service.
Then:
1.
Log into St. John’s Central.
2.
Click on the UIS tab
3.
Click on Click here to Access under the UIS Main Menu Box
4.
Click on Student & Financial Aid
5.
Click on Student Life
6.
Click on Service Reporting System from the student main menu
7.
Click on Individual Service Entry
8.
Select the course (s) and student organization (s). If it is more
than one course or organization, please hold down the Ctrl button
and click the courses or organizations. Please note the course
box will only list courses that you are enrolled in for the current
academic year.
9.
Use the drop down menu to select your site and fill in all
information requested about the site. If your site is not listed,
please email [email protected] to add it to the list.

If you will serve in an elementary school, middle school, or high school, please select Board of Education. Enter the information
on the school where you will serve in the box that will appear.

If you do not have the e-mail address of your site supervisor, please enter [email protected].

Enter the first date of anticipated service and an estimated number of hours you will serve at the site and click Submit.

Then, you will be able to see an edit/preview page of the time log and have the opportunity to make any changes. After
reviewing, you can click Submit.
10. Please return to the main menu to print your Academic Service-Learning time log. Click on Academic Service-Learning Log Sheet and
print form.
11. Make sure to bring the log sheet to the service site every time you serve so it can be signed by your site supervisor. When you completed
your service, log back into the UIS to enter all the additional dates and times and hand in the signed time log to your professor in class.
