Hours in the classroom and clinical attendance are mandated by the Utah Nursing Assistant Registry. Students are allowed a total of three excused absences over the two quarters (1 full semester). Each student is responsible for passing off the required skills before attending clinicals. I will offer 2 skill make-up days, one before school and one after school clinicals for students to pass off the skills. Citizenship Grading This is based upon student's classroom and clinical behavior. An uncorrected series of negative behaviors or one serious incident of negative behavior may result in disciplinary action. Such disciplinary actions may include an inability to attend clinicals, take the state examination, and may result in possible termination from the program. Serious negative behavior includes, but is not limited to conduct such as: 1. Repeated incidents of disruptive or nonproductive behavior 2. Overt disrespect for school authorities or staff members 3. Direct or willful disobedience of school rules/clinical rules or policies 4. Use of vulgarity or profanity 5. Clinical Experience 3 Clinicals dates have not been determined yet, but will be around__________________. I will provide more details and paper work about a month prior to clincials starting. Students will be scheduled specific facilities, dates, and times. Students will be attending several different skilled nursing facilities in Davis County. There will be a Clinical Instructor overseeing the students the entire shift. The student will be paired with a Certified Nursing Assistant in the facility. The student will be participating in patient care. There will be 3 shifts; each shift will be 7-8 hours long. This will include mornings, evenings and one Saturday morning. We will not be attending class during clinicals. Transportation will not be provided by the school to and from clinicals. Sports games, or other important obligations must be given to the teacher prior to scheduling (will provide a date of when I need scheduled by). Students will be practicing skills in lab on a mannequin and some skills will be practiced on another student. Some of these skills include: taking blood pressure, pulse, transferring, assisting with walking, height and weight, assist feeding part of a meal, range of motion, and brushing teeth. This provides the best and most realistic experience prior to taking care of real patients. I have read the class expectations for Mrs. Porter’s CNA class and understand the policies and expectations. I understand the attendance policy and grading policy that is mandated by the Utah Nursing Assistant Registry. ________________________________ Student Signature _________________________________ Parent Signature ________________________________ Student Name Printed _________________________________ Date
