1. The application form once completed should be returned to the QCAA.
2. Applicant should submit the hard/soft copies of the relevant Chapters/extracts of the manuals identified in
item 4, sub section 3.1 in Section Three as attachments.
3. Failure to complete this form in full may result a delay in processing the application.
4. Submitting of this form does not itself constitute an approval for the transport of Dangerous Goods by air.
5. Applicant must complete the compliance checklist in Section Three sub section 3.2.
6. Any amendment to the approved Manual must be submitted from FOD.OAF.007 together with the
compliance checklist in Section Three sub section 3.2 in this Application.
7. This form should be signed by the person requesting the authorization (Post holder).
8. This form is available as a Microsoft Word document to enable it to be filled in electronically and
submitted by e-mail providing attachments can be transmitted likewise.
9. Leave blank the sections which do not apply to your particular application.
