Basketball Camp
August 15-18, 2016
9 am—noon
Bonner Basketball Camp is for Bonner students who are entering 5th through
8th grades. All skill levels are welcome! We are excited to have Ms.
Hollenbeck host the camp. The camp fee will be $10. Please enter the
gymnasium through the playground doors.
Please return this form to the school as soon as possible (but no later than
Wednesday, August 10) to ensure an accurate count and availability. If you
have questions or concerns, please contact the school at 258-6151.
I hereby authorize the instructors of this camp/clinic to act according to their best judgment
in any emergency including those that require medical attention. I hereby waive and release
this clinic/camp and its volunteers from any and all liabilities from injury and/or illness
occurred while at this clinic/camp. I have no knowledge of any physical impairment that
would affect the student’s participation in this camp/clinic at Bonner School.
