Documents marked with ** should be issued within the past 12 months. Not denoted – it can be more than 12 months old. DBS Application Guidance Disclosure of Criminal Convictions 8. Please remember to read and sign the declaration on the back of the form and bring the form along with evidence requested to your interview. The job you have applied for is exempt from the Rehabilitation of Offenders Act (1974) so you must tell us about all non protected spent or unspent convictions, cautions, warnings or reprimands you have. In completing the Disclosure of Criminal Convictions form you are agreeing to Gateshead Council performing a DBS check on your record. Verifying Identity Completing the Form Route One The form should be completed in black ink and block capitals. You must provide a document from Group 1 and 2 further documents from group one or 2a or 2b; one of which must verify your current address. You must complete all the fields on the form that are applicable. This guide will help you to provide the evidence to accompany your form in order that we can process your application efficiently. 1. You must complete your title and name in full and complete a continuation sheet if you have had any other surnames or forenames. We will need proof of your name and proof of any changes such as a marriage certificate or deed poll. There are three routes that we must follow to verify your identity. Non – UK/ Non EEA Nationals and EEA nationals living in the UK for less than 5 years must be validated through route one. Non EEA nationals should be validated via Route One only. If we cannot validate your identity by using Route 1 we must use Route 2. Route Two 2. You must give a full 5 years address history using the continuation sheet if necessary and remember that dates to and from must run concurrently e.g. From: 07/2008 To: 04/2010 From 01/2002 To: 07/2008. You will need to provide proof of your current address. 3. Please complete all date and place of birth fields and provide proof of your date of birth. 4. If you have a National Insurance Number you must write this on the form. 5. If you have a driving licence you must write the number on the form. 6. If you have a passport you must write your passport number on the form. 7. If you already have a Scottish vetting and barring number you must write this on the form. The applicant must produce 1 document from Group 2a AND 2 further documents from Group 2a or 2b; one of which must verify their current address. We will then use an external ID validation service to establish your name and living history footprint. If the applicant fails the validation check they will need to be processed through route 3. Route Three We must ensure that Routes 1 and 2 are exhausted before we consider validating identity using route 3. The applicant must produce a certified copy of a UK birth certificate AND 1 document from Group 2a AND three further documents from Group 2a or 2b; one of which must verify their current address. If the applicant fails to produce the evidence for Route 3 validation they will need to go for fingerprinting which will delay the application process.
