EXCURSION TO CANNINGTON (GOOD GAMES & BOUNCE) INFORMATION FORM FOR PARENT/GUARDIAN To be retained by parent Year 5 & 6, Room 2: 5th December 2016 REASON FOR EXCURSION The Year 5 and 6 cohort has been working towards a trip by demonstrating the four elements of the Tribes agreement. Active Listening, Appreciation, Mutual Respect and Participation. The ‘Bounce’ activity enables them to harness those elements and apply them in a social context. They have also been studying and developing chance experiments and the language of instructions and procedures. The ‘Good Games’ activity will allow them to apply their skills in reading and following instructions in procedures and experience first-hand the elements of chance, probability and statistics. The train journey will also give us chance to study timetables and scheduling. ACTIVITIES TO BE CONDUCTED ACTIVITY & TIME 8.50 Gather students outside Room 2 and begin walking to Bayswater Train Station 9.00 Arrive at Bayswater Train Station to meet Transperth representative 9.06 (alternative if necessary) Depart on city bound train on platform 1 (The train stops in Perth City and then carries onto Cannington station on the ARMADALE LINE) 9:29 Arrive at Cannington Train station and walk to Bounce (Alternative train is planned if we miss the first. 10:30 Meet Bounce staff and enjoy a 1 hour session of Bounce, finishing at Midday. 12:00 Finish Bounce session and walk to Good Games Cannington. 13:45 Finish Good Games session and walk back to Cannington Station 14:00 Meet with Transperth representative and board 14:10 train (The train stops briefly in the city, and a change will be made onto the MIDLAND LINE) 14:42 Arrive at Bayswater Train Station 15:00 Home time COST Bounce -$10 per student Train - $2 per student Good Games - $100 flat rate The costs will be fully paid for by Muffin Morning funds. LOCATION Bounce Cannington, 371 Albany Highway, Cannington 6107 Good Games Cannington, 21 Liege St, Cannington, WA 6107 CONSENT FORMS AND ACCOMPANYING PARENTS Please sign and return the attached “Parent/Guardian Consent Form” to your child’s teacher by Friday 26th of November. Parent helpers are required to accompany the class on the excursion – please indicate your availability on the consent form and advise your class teacher. Parent helpers may not be assigned to the same group as their own children. This will be at the teacher’s discretion and discussed with parents prior to the excursion. TRANSPORT ARRANGEMENTS All students, teachers, assistants and parent supervisors will travel by train to and from Cannington. Students', teachers', parents' and caregivers' train ticket costs will be covered by the school. STUDENT CONTACT ARRANGEMENTS DURING EXCURSION Urgent messages can be relayed via the school office on 9370-2465. SUPERVISION TO BE PROVIDED Students will be supervised by class teachers and accompanying parents. STAFF ACTION IN CASE OF ACCIDENT OR ILLNESS ON THE EXCURSION Supervising teachers will carry a mobile phone, with a list of parent contact phone numbers, and any medication required by students. They will carry the school first aid kit. SPECIAL CLOTHING OR OTHER ITEMS REQUIRED Students should wear their school uniform, including closed shoes or boots. They need to bring a water bottle, a snack for recess and their lunch in a clearly labelled disposable bag or lunch box. NOTE: Staff accompanying students on excursions will take all reasonable care while the students are in their charge to protect them from injury and to control and supervise their behaviour and activities. Parents/guardians should be aware that staff members are not responsible for injuries or damage to property which may occur on an excursion where, in all circumstances, staff have not been negligent. In the case of excursions not involving an overnight stay, costs incurred as a result of accident or illness is the responsibility of the parent/guardian. Parents are required to inform the organisers well before the scheduled excursion departure of any change to their child’s health and fitness so that appropriate supervision may be arranged. Where it is considered necessary, school staff will arrange medical assessment and treatment for students.
