Welcome to the 2017 ORJT Football and Cheer Season!
ORJT strongly encourages all interested participants to apply, regardless of
previous coaching experience. As a community organization, ORJT exists only
on the volunteer participation of our parents and we need dedicated and loyal
parents that want to serve our children. Our mission is safety and having fun.
If your #1 priority is for the kids to have a positive, enriching life long
experience, then we want you to apply!
Instructions:
1. Please fill out the Football and Cheer Head and Assistant Coach
Application completely.
2. Applicant Information:
a. Please be sure to provide at least 1 phone number, either home or
cell. The information you list on the application will be used for
the ORJT roster.
b. Please list an email address. ORJT communication will be sent via
email, text, and/or the ORJT website.
3. Indicate “yes or no” if you live in the ORHS boundary. It’s not
mandatory for a coach to live within the ORHS boundary.
4. Applicant Position and Division of Interest
a. Please put an “X” next to the group you are applying for, either
Football or Cheer.
b. Please write “Yes or No” next to Head Coach and “Yes or No” next
to Assistant Coach for the team/squad you are interested in.
c. If you are applying for multiple teams/squads, please indicate #1,
#2, #3, etc., for your order of priority.
5. Coaching Experience
a. Please list all of your relevant coaching experience (if any) in youth
sports with the name of the organization, your position, the dates
and main duties.
6. What does coaching mean to you? Please give us the main reason you
would like to coach for the kids of ORJT.
7. References
a. Please list the name and phone of at least 2 personal or business
references. They do not have to be part of ORJT.
8. ORJT Coach/Board Agreement – please sign the agreement upon
submission of your application.
9. Please print, scan and email page 2 of your completed application and
the ORJT Agreement to - @ [email protected].
10. - Ensure you cc the VP of Cheer or Football depending on what
