Introduction
Have you just had a baby?
Complete the Birth Registration Statement Form provided by the hospital.
How to lodge your birth registration form
You can lodge it in the following two ways:
1. Post to:
Access Canberra
GPO Box 158
Canberra ACT 2601 or
2. Lodge in person:
At an Access Canberra Service Centre located in Belconnen, Gungahlin, Tuggeranong
or Woden.
Getting a birth certificate
To apply for a birth certificate at the same time, complete the ‘Birth Certificate Order’
section of the Registration Statement Form.
Current fees and processing times are available by searching ‘birth registration’ at
www.act.gov.au/accessCBR (Note: Gungahlin and Belconnen Services Centres are electronic
payments only.)
Requests for certificates lodged by post can be paid for by credit card, cheque or money
order.
Birth certificates will be sent by registered post to the person listed as the informant on the
Registration Statement Form.
