OUTCOME MEASURES:
NHS Outcomes Framework
The NHS Outcomes Framework provides indicators that can be measured at CCG level
together with additional indicators developed by NICE and the Health and Social Care
Information Centre. These provide clear, comparative information for CCGs, Health and
Wellbeing Boards, local authorities and patients and the public about the quality of health
services commissioned by CCGs and the associated health outcomes. They are divided into
5 over-arching domains:





Preventing people dying prematurely
Helping people recover from episodes of illness or injury
Ensuring that people have a positive experience of care
Enhancing the life of people with long-term conditions
Treating and caring for people in a safe environment and protecting them from
avoidable harm
There are a number of specific measures sitting beneath each domain.

This framework sits alongside an outcomes framework for adult and social care and whilst
the distinct frameworks reflect the different delivery systems and accountability for the NHS
and social care, they are now further aligned to encourage collaboration and integration.

The Better Care Funding programme also requires an integrated approach to reporting and
agreed outcome measures are as follows:





Permanent admissions of older people (aged 65 and over) to residential and nursing
care homes, per 100,000 population
Proportion of older people (65 and over) who were still at home 91 days after
discharge from hospital into reablement / rehabilitation services
Delayed transfers of care from hospital per 100,000 population (average per month)
Avoidable emergency admissions (composite measure)
Patient / service user experience
As a South Tees community, we have also developed a ‘dashboard’ of metrics in order to
track progress and highlight any potential blockages or issues within the system which
require further investigation.
