•Are 18 years or older, or moving from Children’s to Adult Services
•Who live in or are from the London Borough of Hammersmith & Fulham
We can help you:
make choices
be healthy
visit your doctor and other health services
communicate
eat and drink safely
move about
be more independent
have a Health Action Plan
look after yourself, for example, with budgeting and personal care
get a freedom pass, direct payments, or benefit advice
use residential or nursing care if you need it
get training and a job
meet other people and take part in activities
take short breaks for you and your carer(s)
We can also give advice, support and information
to your family, carer and partner
