Once approved, the parent can print the VPK certificate from any printer by accessing their Family Portal account. examples: Birth Certificate OR passport OR an immunization record signed by a public health officer or by a licensed physician They will need their email address and password to log into the Family Portal account. Providers can assist families by: Obtain a blank “Provider Transfer Form” from any of our ELC eligibility offices or by downloading the form from our website at www.elcpalmbeach.org/child-care-resource-and-referral-forms Submit a completed “Provider Transfer Form” to any our ELC eligibility offices. No Back Dating of Certificates- ELC will not authorize care prior to the date that the “Provider Transfer Form” was submitted. Please allow 2-3 business days for processing. New certificates will be sent to the provider via “BOX”. Original certificates will be mailed to the parent at address on record. Family Services School Readiness Transfer Requests continued: Families Leaving your Program: Determine if the parent has any outstanding fees. If the parent has outstanding fees, please work with the parent to resolve the matter. If the parent has no assessed parent fees or a zero balance, complete the bottom portion of the “Provider Transfer Form” and return form to parent. Remind parent that ELC requires 2-3 days to process the request. Remind parents that a new certificate will be needed prior to starting the child at the new facility. Families Interested in Transferring to Your Program: Assist parent with completing the transfer process by providing a blank copy of the “Provider Transfer Form”. Educate Parent about the Transfer process and timeframe. (2-3 business days) Review new certificate to ensure that your site is listed and for authorization start date to ensure proper payment. Family Services Funding Notification Update ELC monitors the funding utilization monthly. We are not currently enrolling children from our Wait List or are processing any new CSC or Refugee Referrals. We continue to process any At-Risk, Career Source and Teen Parent referrals. Families are encouraged to apply for any available funding or place their name on the wait list a www.elcpalmbeach.org Family Services Revalidation (Updating the Wait List Status) A parent whose name appears on the Waiting List of the Early Learning Coalition for child care services must update their information every six (6) months in order to remain on the list. Families can update their status online at www.elcpalmbeach.org Step 1: Go to www.elcpalmbeach.org Step 2: Click the green scroll at the top of the page to apply for VPK/School Readiness Services. Step 3: The family logs on to the Family Portal using their user ID number. Step 4: Click Apply for School Readiness and complete/edit the questionnaire. Step 5: Upload the required documents. Step 6: Submit your application update. Step 7: You will receive an email notification once your information has been updated. Being placed on the waiting list does not guarantee acceptance into the subsidized child care program. Families are notified if funds become available to provide subsidized child care services. Provider Reimbursement Monthly Attendance Submission Due Date Attendance documentation submission (item # 48, page 10 of the SR20 Form – State Contract). Provider agrees to submit monthly attendance reports for payment. Provider agrees to submit all required attendance records to Coalition on or before the 3rd (third) business day of each month. If the due date falls on a holiday, Provider agrees to submit all required attendance records to coalition on the preceding business day. Records submitted late will be processed and paid in the next open payment cycle.
