Medication & Immunization 2017-2018 DSST must follow state and district guidelines regarding medications. The district requires that “over-the-counter” nonprescription drugs be addressed in the same manner as prescription medications for students at school. DSST must have its assigned Denver Public School Nurse onsite before the school can accept any medications. The DPS Nurse is typically assigned by mid-August. The DPS Nurse will begin reviewing the submitted health documents and contacting families as needed at that time. The school cannot accept any medications, and students may not bring medications to school without the required documents fully and accurately completed, signed by the appropriate parties, and on file at the school. The information on the signed medication form must match exactly the information on the medicine container. The school may not make any exceptions to this policy. If the school has not received the appropriate signed documents and/or if the Denver Public Schools Nurse has not delegated a specific medicine administration responsibility to appropriately trained school staff, the student’s legal guardian must administer the medication to the student if it is needed at school. Please answer the following questions to determine whether or not you need to provide us with a medical form. All forms listed below can be found at dsstpublicschools.org/stapleton-middle-school under the “2017-2018 Schoolyear” option under the “Resources” tab. 1. Have you already turned in proof of immunization and your child is up to date with immunizations and your child has not received new immunizations since the last time you submitted your immunization proof? Please refer to the letter from the Colorado Department of Public Health & Environment for more information. Yes If yes, you do not need to take further action unless the nurse contacts you with questions. No If no, please submit a current copy of your child’s proof of immunizations during registration. If your child is not up to date with their immunizations at registration, be sure to make arrangements for your child to receive the appropriate immunizations prior to the 1 st day of school. The Denver Public Schools’ nurse will be contacting families for whom we do not have proof of up to date immunizations at the beginning of school to advise them of needed immunizations. 2. Does your child need to bring asthma medication to school? Yes If yes, you must complete the “Colorado Asthma Care Plan” form, have your child’s physician complete and sign the appropriate portion of the form, and then submit the form to the school. Your child must have the completed and signed form on file prior to bringing medication to school. No 3. Does your child need to bring medication to school for severe allergies and anaphylaxis reactions (e.g. an EpiPen or similar medication)? Yes If yes, you must complete the “Colorado Allergy and Anaphylaxis Emergency Care Plan and Medication Order” form, have your child’s physician complete and sign the appropriate portion of the form, and then submit the form to the school. Your child must have the completed and signed form on file prior to bringing medication to school. No 4. Will your child need to bring any other type of medication to school, or have the school administer any other type of medication to your child (this includes over-the-counter medication such as Tylenol)? Yes If yes, you must complete the “Denver Public Schools Student and Medication Request Release Agreement” and sign the form, have your child’s physician complete and sign the appropriate portion of the form, and then submit the form to the school. Your child must have the completed and signed form on file prior to bringing medication to school. No 5. Will your child be participating in any team and/or club sports? Yes If yes, you must complete the “Physician Release and Permission” form, have your child’s physician complete and sign the appropriate portion of the form, and then submit the form to the school. Your child must have the completed and signed form on file prior to beginning a sport. Students only participating in general physical education class are NOT required to have a physical on file. No
