Nursing Program Weighted Admission Score Sheet Generic and Advanced Placement Instructions Complete and submit this score sheet to the Division of Health Sciences along with the program application checklist to receive a Health Sciences Nursing program application. Second preference given to residents of other South Carolina counties, and finally to applicants from other states. c. Preference is given to those students who have not previously been admitted to the Nursing Program. d. Students applying for Licensed Practical Nurse (LPN) to Associate Degree Nurse (ADN) entry must meet separate criteria, please meet with the nursing program director. II. TEAS test score: a. Good for 3 years prior to application. b. The TEAS test may only be taken twice within a 12 month period from date of first test. c. Only the highest score will be considered. d. ALL TEAS Test Results must be included (not just those taken within the past 12 months). Criteria Considerations Scoring Grade Points 1.
