Who Qualifies: The rebate is available to anyone new to a product in the DAILIES® brand family of contact lenses.
$200 Rebate Example: A patient transitions From Monthly/Weekly or DAILIES® Sphere to DAILIES® Multifocal lenses.
$120 Rebate: If a current DAILIES® wearer opts to stay in that exact same product.
Focus® DAILIES® contact lenses are not included in the a rebate program.

2) COMPLETE Registration
3) VIEW/DOWNLOAD Rebate Form (Also Available Via Email Link per email in registration)

a. Electronically Upload OR Mail the following required documents:
i. Original sales receipt with DAILIES TOTAL1® or DAILIES® AquaComfort Plus lens product circled.
ii. One (1) Original UPC code for the brand of lens purchased.
iii. Copy of eye exam receipt and/or contact lens fitting receipt with date circled.
iv. Your completed official Alcon rebate form (provided at www.myalconlensrebates.com)
