Homelessness Partnering Strategy COMMUNITY ENTITY METRO VANCOUVER 2016-2019 Project Investments (as of January 13 2017) The Metro Vancouver HPS Community Entity works in partnership with Service Canada and the HPS Community Advisory Board (CAB), on calls for proposals, submissions review and funding decisions. The CAB sets the terms of reference for each call for proposals and directs the allocation of funds within the framework of the HPS program. The 2014-2019 HPS program requires a minimum of 65% of funds be awarded to Housing First projects. The CAB may allocate the remaining 35% to other HPS priorities and activities in Capital, Support Services, and Regional projects. The following is a preliminary list of HPS-funded project investments. Additional supports, services and resources, as directed by the client, through an intensive case management service. (2014-2016 project offered Housing First funds up to March 2017) $60,795 Provide case management & individualized life skill supports to chronic/episodic homelessness; case management services to assist with successful tenancy. (2014-2016 project offered Housing First funds up to March 2017) $457,028 For Housing First eligible seniors 55+, and one part-time outreach worker specific to Housing First eligible Francophones 19+, on the North Shore; to place clients in permanent housing. $1,639,267 Expansion of Housing First program and hours of service to provide housing for 120 clients. 7 RainCity Housing & Support Society LGBTQ2S* Youth Housing First Program $864,543 Housing first project located in Vancouver that will provide housing and supports to chronic/episodic homeless youth who identify as Lesbian, Gay, Bisexual, Trans, Queer and Two Spirit. 8 Stepping Stone Community Services Langley Outreach Housing First $332,609 Housing first case manager to work with homeless outreach team to provide services for clients who are identified as episodically and chronically homeless, using a housing first approach. Stepping Stone established a Housing First Advisory Committee of community partners. 9 Aunt Leah's Independent Life Skills Society The Link Housing First 10 Alouette Home Housing First Start Society ICM Team for (AHSS) Maple Ridge, Pitt Meadows, & Katzie First Nation RainCity RainCity Surrey Housing & ICM Team Support Society $546,164 The Link Housing First began operation in January 2015 in partnership with HPS and BC Housing and serves the needs of homeless foster youth who have, or will soon, ‘age out’ of provincial foster care system. This housing first initiative focuses on youth from foster care who are experiencing chronic and episodic homelessness. Program brings a preventative aspect to housing first model, to prevent aging out into a street and shelter homelessness. $324,964 ICM team for region, with HPS funding case manager and Pending housing specialist (focused on landlords), goal is to house new 20 clients on average taking a month to move in from information assessment. Meet bi monthly with partners, have subsidies. Internal and external evaluation. 11 $1,271,398 Serve clients and provide scattered site housing to people who are chronic/episodic or those with a history of chronic/episodic homelessness in Surrey/White Rock as primary location and Langley as secondary location. Place 15 clients with complex addiction issues into permanent housing. $3,100,137 A collaborative between E Fry, Options, Lookout and Sources in Surrey serving chronically and episodically homeless or history of either; goal to place 157 clients. 12 The Elizabeth Fry Society of Greater Vancouver Surrey Housing First Collaboration Hitting The Nail on the Head 13 Pacific Community Resources Society Broadway Youth Resource Centre Housing Outreach $502,114 Partnering with the Broadway Youth Resource Centre Collective, will house 30 youth aged 0-24 and include Aboriginal community, youth transitioning to adulthood and LGBTQ2S community. 14 RainCity Housing & Support Society Coquitlam Housing First Case Management $257,210 To provide scattered site Housing First case management to up to 15 people. Case manager will provide Housing First supports to chronic and/or episodic homelessness and who have significant and complex issues with a focus on people 55+. 15 The John Howard Society of the Lower Mainland From Prison to Community Providing Housing First $655,415 Provide wraparound services and connect individuals to permanent housing for successful reintegration into the community. Provide in-reach at correctional centres to facilitate the intake process at the earliest opportunity and identify Housing First eligible individuals. Develop release and aftercare plans based on personalized needs and provide outreach services to clients upon release with focus on stable housing. SUPPORT SERVICES – PRIORITY: to improve the self-sufficiency of homeless individuals and families and those at imminent risk of homelessness through individualized services 16 Circle of Eagles Lodge Society Housing and supports $279,209 Provide housing and support services; goal to serve 26 clients; 13 housing placement and 13 loss prevention. 17 Covenant House Vancouver Right of Passage $363,852 Supportive transitional housing program, providing 25 bachelor suites to youth ages 16-24 who are homeless or at imminent risk of homelessness; provides youth with a structured 6 step life skills program, support with individualized goals and plans, access to educational and employment resources, housing placement, and mental health and addictions counselling. 18 Sources Community Resources Society Essentials to Housing $549,762 Support project serving the general homeless populations of Surrey, White Rock, and Langley; place clients into stable housing, while their main focus will be in housing loss prevention. 19 Cwenengitel Aboriginal Society Indigenous Services for Indigenous People in Surrey 20 Pacific Community Resources Society Surrey Youth Housing and Homelessness Prevention Program $232,739 Serve Aboriginal clients between ages of 16 and 65 in Surrey. While Cwenengitel currently has a residential program serving 14 clients, HPS funding will allow them to extend their services to non-residential clients and provide cultural activities, housing placement, and counselling. $239,457 Fund staff wages to provide direct client support to youth and young adults up to age of 24 who are homeless or at imminent risk of homelessness in the Surrey/White Rock area; place 125 clients into more stable housing and assist 125 clients with loss prevention services. 21 Vancouver Aboriginal Transformative Justice Aboriginal Housing Support Program $483,448 House 60 clients per year; work with Aboriginal individuals who are homeless or at imminent risk of homelessness with a culturally based approach of Housing Circles. Upgrades to the kitchen, bathrooms, electrical, plumbing, and venting & space configuration, including offices & office set-up to enhance this facility, allowing for extended hours to better serve the women of the community who have repeatedly called for a 24-hour low-barrier, drop-in, and women-only shelter space. 29 Young Women's Christian Association Fire Hall No. 5 Transitional Housing - 15 units $259,285 Construction of 31 unit building, including 15 HPS funding units comprising 36 transitional beds for homeless and at imminent risk of homelessness. 30 Atira Women's Resource Society 41 East Hastings - 52 units $1,000,002 Facility construction of 198 unit building including 52 HPS funded units comprising 52 units of permanent supportive housing for women who are chronically or episodically homeless or at imminent risk of homelessness 31 Atira Women's Resource Society $250,000 New housing from upcycled shipping containers to provide safe, affordable long-term housing to women and children. 21 beds will be created with HPS funds. 32 Lu'ma Native BCH Housing Society 420 Hawks Avenue - 21 units (Converted shipping containers) 950 Main Street - 30 new units 33 The Elizabeth Fry Society of Greater Vancouver Surrey Project Transitional Housing - 25 housing units $762,642 New construction for episodically homeless women; 25 long term housing units $800,625 Construction of 30 new permanent supportive housing units for Aboriginal homeless or at imminent risk of homelessness. REGIONAL Non-Housing First - PRIORITIES: to ensure coordination of resources and leveraging and to improve data collection and use 34 Sources Community Resources Society Ask An Advocate 35 Greater Vancouver Shelter Strategy Supporting Partnerships between Health & Homelessness $375,967 Website provides online advocacy support to organizations serving Metro Vancouver’s homeless population; will include 30 tutorials and a Tenancy and Income Supports Advocate available for questions online. To increase community (regional) capacity to address homelessness. Coordinate and provide access to tenancy and income support advice for organizations supporting individuals who are homeless or at risk of homelessness to obtain and maintain permanent housing. A regional forum for communications, tools, and training. $312,264 Enhance health support to people experiencing/exiting homelessness in their transitions to shelter and housing in collaboration with Fraser Health, Providence Health Care and Vancouver Coastal Health and SFU Gerontology Research Centre. To understand resources needed to support transitions from hospital to shelters and housing, evaluating two projects on health supports and conducting a cost benefit analysis of hospital stays. Building on the Bentwood Box Housing First Readiness project and Metro Vancouver Community Entity’s five Housing First Readiness projects – for Aboriginal organizations serving Aboriginal homeless persons. $11,440 Compile relevant best practices / model solutions (regionally, nationally, internationally) for addressing practices and solutions for Regional Homelessness Task Force pathways into homelessness, in particular effective prevention and diversion practices; addressing the needs of those who are already homeless, including emergency shelter, outreach, drop-in and meal programs; creating pathways out of homelessness, such as Housing First, employment programs, support services for mental health and substance use issues.
