A nutrition incentive program where healthcare providers offer clinical care, nutrition education, and/or a fruit and vegetable prescription redeemable for fruits and vegetables at a participating retail location. Distribute the survey to the patient and provide interpretation support when needed. 2. Check that the program ID is on every page of the survey. 3. Collect completed surveys and make copies of the surveys and keep the originals in a secure file on site. 4. Enter survey data into DATABASE NAME RETAIL Training THE PRESCRIPTION THE PRESCRIPTION WHAT IS THE PRESCRIPTION WORTH? For example: $1 per household member x 7 days per week x 4 weeks = $28 per month This family’s prescription is worth $28 per week or $112 per month. RETAIL Training QUESTIONS? Participant uses alternative currency to purchase fresh fruits and vegetables from vendors COLLECTING REDEMPTION DATA The Redemption Log is to be used to collect the following information: • Participant ID • The total $ amount of tokens distributed • The total $ amount spent at the market The market manager/coordinator will use the Farmers Market Online Spreadsheet to report on redemption data.
