CLINICAL PROTOCOLS
• Enroll TARGET POPULATION patients to participate in the
program
• Obtain patient written consent to participate
• Conduct TOTAL NUMBER & FREQUENCY OF VISITS clinical
visits with patients
• Educate participants on the importance of fruits and vegetables
in their diet
• Ensure that the provider and nutrition visit take place on the
same day
• Collect health indicators during each FREQUENCY clinical visit
• Disburse prescriptions to patients during FREQUENCY visits
• Report clinical data on a FREQUENCY basis using NAME OF
DATABASE

surveys and keep the originals in a secure file on site.

• Collects data/records use on prescription
• Returns prescription to participant or retains a fully
redeemed prescription
• Distributes alternative currency (e.g. tokens)
4. Participant uses alternative currency to purchase
fresh fruits and vegetables from vendors
COLLECTING REDEMPTION DATA
The Redemption Log is to be used to collect the
following information:
• Participant ID
• The total $ amount of tokens distributed
• The total $ amount spent at the market
The market manager/coordinator will use the Farmers
Market Online Spreadsheet to report on redemption
data.
