Every team member has signed an Individual Registration Form. Attach these
forms to your Team Registration Form before submitting form
Team Registration Form is in alphabetical order (by last name)
(Helps to ensure accurate tracking of miles throughout the program)
Names on the Team Mileage Form are in the same order as on the Team
Registration Form
