1 – Fill in the team roster (if not already)
- Place a checkmark next to each ABSENT
player
3 – Enter team names in
Shots For and Score areas
2 – Fill in
scorekeeper
and ref names
Note that each shot on net is
recorded during the game with a
mark in the appropriate period box
When a Goal is Scored
1 – Enter the name of the team, goal scorer,
assists (if any), period, and clock time  Enter
Full names or at minimum Name + Initial
(Donald Duck is best, D. Duck and Donald D. are okay)
- Add in Notes for powerplay (PPG),
shorthanded (SHG), penalty (PSG), or empty
net (ENG)
2 – Add marks next to roster
names for goals and assists
3 – Place a mark for the score in the
period box
- Remember to add a shot to the
shot tally
At the End of the Game
3 – Mark the three stars of the
game (1 star = best)
2 – Tally the
1 – Add up total shots for each
final score
period and total for the game
4 – Add any random notes
about the game (rain delay, amazing
goal, fight broke out, unruly fans…)
