About JackDawe
The JackDawe Team is a
specialist Home Care service,
providing tailored individual care
and support for people with
dementia in their own homes.
The service works in partnership with
Nottingham City Care, having direct
access to occupational therapists and
mental health nurses.
The aim of the team is to work with
citizens and their carers to develop
a care package that promotes and
encourages their independence.
The care plan focuses on how we can
best support them and help meet their
needs. As a person’s needs change
over time so will the care plan, with
regular and ongoing reviews.
JackDawe is a city-wide team,
catering for a diverse population
including a focused Men’s Team,
Polish Team and Sahara Team,
catering for South Asian women.

Key indictors for professionals when
considering if JackDawe support is
appropriate:
• Individuals must have a diagnosis
of dementia from either the GP or
secondary services
• Individuals must live within the
boundaries of Nottingham City
• The individual has or requires
specialist multi-disciplinary
case management to support
with mental health/behavioural
conditions
Support pathway
JackDawe Assessment Team receive
referral via the service organisers
6 – 12 week assessment by the team of
the citizen’s health and needs
• There is an opportunity to
maximise, enable and maintain
activities of daily living
• Situations where there are
significant risks posed by the
individual or they are at risk of
harm from others
Support continues with
the JackDawe Home Care
Service
Case transferred to
independent sector provider
or individual admitted to
residential care
• Situations whereby a flexible
approach to support is required
due to a history of nonengagement with services
After the initial assessment period the
care package will be reviewed and
a joint decision will be made as to
whether JackDawe remains suitable
for the citizen or we move their care
to another service provider.

All cases are subjected to regular multidisciplinary reviews.
The citizens may remain within the service unless it is deemed
that their care needs can be met by an independent sector
provider or where home care is no longer appropriate and
residential care is required.
