1 Job purpose
To support individual and/or their families in their own homes to manage aspects of
daily life.
Their work will focus on maximising individual’s independence and enabling them to
link back into their communities.
The post holder will carry a personal caseload and duties are listed below.
2 Principal duties and responsibilities
1 Undertake introductory visits to new or re-referred service users, including a sharing of
information about the Home Care Service, undertaking environmental checks and
reporting to the Home Care Manager on any relevant health and safety issues or
significant or potential problems in the working environment.
2 Report to the Home Care Manager, providing written and verbal reports on service issues.
Compile relevant and accurate information to assist the Home Care Manager to maintain
and progress service provision.
3 Develop individual care plans for service users that will enable them to regain
independence and reable them back into their communities.
4 Undertake reviews of service users’ care plans, update information and take a lead in
implementing changes to service users’ service provision.
5 Feedback, monitor and update on the progress and needs of the service users with the
relevant Home Care Manager.
6 To input IT systems to input service user information.
7 Provide direct care to service users in accordance with the job description for Home Care
Workers.
8 Work closely with other agencies in order to achieve optimum independence for individual
service users.
9 Attend case conferences, transfers and other meetings as required.
3 All staff are expected to maintain high standards of customer care in the context of

and safety standards and to participate in training activities necessary to their post.

Undertake work on a rota system in the event of
