Application for Workplace Provider/Employer Accreditation
Guidelines
Purpose
The purpose of Workplace Provider/Employer Accreditation is to ensure that an employer meets the
FP&M SETA quality assurance criteria when training internally. All employers providing learning
and/or assessment within the FP&M SETA ETQA scope will be classified and managed in terms of the
FP&M SETA Accreditation Policy.
Step by Step Process
The following steps apply to the licensing process:
STEP 1
Employer applies for Workplace Provider Accreditation




Employer makes initial contact with ETQA division.
A Workplace Provider application form is made available to the employer.
The employer completes the application form and returns it with the necessary attachments to
the Regional FP&M SETA Office.
The application form consists of the following sections:

STEP 2
Desk review




The application is reviewed by the FP&M SETA ETQA.
If the desk review is favorable, the employer is informed and a site visit of the employer is
scheduled by the ETQA division. FP&M SETA ETQA will confirm date and time of the site visit
in writing to the employer.
If the desk review is unfavorable, the employer will be informed in writing. An unsuccessful or
incomplete application will be sent a developmental compliance plan within time frames.
If the application is rejected the employer will be advised in writing stating the reasons. The
employer will have the right to appeal.
STEP 3
Site evaluation


The FP&M SETA Audit team conducts the audit at the premises of the employer.
The audit will include a physical inspection of learning and assessment materials along with an
inspection of all the facilities required for the implementation of the learnership/apprenticeship.
STEP 4
Final Audit Report





The final audit report is completed by the ETQA team and submitted to the ETQA committee for
ratification.
The final results of the audit, including recommendation and decision regarding licensing status
are conveyed to the employer.
Should an audit result be unsatisfactory, the employer will be given a developmental
compliance plan with agreed timeframes to meet the non-compliance issues.
Should an audit result be favorable, the organisation will be granted FP&M SETA employer
licensing for a period of five years.
The successful applicant will receive a FP&M SETA Employer License Certificate.
Notes
The employer will have full right of appeal as described below:
1. Should any applicant be dissatisfied at any point in the proceedings, they have the right to
communicate such dissatisfaction to the ETQA Manager of the relevant region of the FP&M
SETA directly
2. Any person aggrieved by the decision of the FP&M SETA may appeal according to the appeals
procedures of the FP&M SETA in writing. The executive committee shall deal with such an
appeal transparently.
3. The ETQA Manager shall periodically arrange for unannounced follow-up evaluations to be
carried out on accredited workplace providers/employers and make situation reports thereon.
The licensed organisation/workplace will be advised in writing of the result of such evaluation
through a developmental compliance plan.
Please note that if you are currently paying levies to another SETA and your scope falls within
that of the FP&M SETA, you will first have to complete a change of SETA form before
continuing with this process.

SECTION D
TERMS AND CONDITIONS
The organisation agrees to the following terms and conditions:
1. That should the organisation be deemed eligible for employer licensing, the organisation
undertakes to participate fully in the Workplace Provider/Employer Accreditation process.
2. That the applicant will supply all information, pertinent to the accreditation application and
process, freely and without prejudice to FP&M SETA.
3. That all information supplied to FP&M SETA will be verifiable, true and accurate.
4. That the applicant will inform FP&M SETA of any change to the information supplied in this
application or subsequent supporting documentation.
5. That should the applicant, or any of the educators / trainers / instructors / assessors in their
employ, fail to comply with any rules, regulations, or conditions of employer licensing as
stipulated by FP&M SETA, the FP&M SETA reserves the right to suspend and/or refuse the
licensing of the applicant forthwith. Suspension/refusal of employer licensing will be
communicated to the applicant, in writing, citing the reasons for the decision.
6. That the applicant will have access to an appeals policy and procedure should the application for
licensing be suspended or refused, by FP&M SETA, at any stage of the process.
