PLAYER REGISTRATION FORM 2016-2017 SEASON
It is very important that this form is fully completed and returned to the Registration Secretary, with a passport size photo of the
player attached.
If the player has never played in this league before, you must provide evidence of the player’s date of birth.
i.e. a photocopy of original birth certificate, a photocopy of the player’s passport or other official document issued by a Government
Agency attesting to the player’s date of birth.
Until the Registration Secretary receives a passport size photo and if required proof of player’s age, the player will not be deemed to
be registered for his/her team.

By Signing this form, you agree that all details above are true and correct and that you agree to your details being entered onto the
League Website. (Players details are password protected and can only be accessed by club officials with the leagues consent)
You must make sure that all the required signatures are completed by the individuals. If they are not, the registration form will not
be accepted and will be returned to the Club Secretary.
If any Parent, Guardian or Club Official signs the form knowing that the player is signed on by a Professional Club as either Academy
or School of Excellence, the player will have his / her registration cancelled for the season and the Club be dealt with under rule. It
is the Club’s responsibility to notify the League of any player who is signed on by a professional Club during any season.

Parents / Guardian and supporter’s guidelines for RESPECT
Spectators and parents/carers Respect Code of Conduct WE all have a responsibility to promote high standards of behaviour in the
game.
This League is supporting The FA’s Respect programme to ensure football can be enjoyed in a safe, positive environment.
Remember children’s football is a time for them to develop their technical, physical, tactical and social skills. Winning isn’t
everything.
Play your part and observe The FA’s Respect Code of Conduct for players at all times.
I will:
• Remember that children play for FUN • Applaud effort and good play as well as success.
• Always respect the match officials’ decisions
• Remain outside the field of play and within the Designated Spectators’ Area (where provided)
• Let the coach do their job and not confuse the players by telling them what to do.
• Encourage the players to respect the opposition, referee and match officials.
• Avoid criticising a player for making a mistake – mistakes are part of learning.
• Never engage in, or tolerate, offensive, insulting, or abusive language or behaviour.
I understand that if I do not follow the Code, any/all of the following actions may be taken by my club, County FA or The FA.
I may:
• Be issued with a verbal warning from a club or the League.
• Be required to meet with the club, League or CFA Welfare Officer.
• Be required to meet with the club committee
• Be obliged to undertake an FA education course.
• Be obliged to leave the match venue by the club Referee or League Officials.
• Be requested by the club not to attend future games
• Be suspended or have my club membership removed.
• Be required to leave the club along with any dependents. In addition: • The County FA could impose a fine and/or suspension on
the club as well as the ADJFL.
