Position Openings: CARES, Benton Harbor Location The purpose of the following job openings is to focus on linking and retaining HIV positive individuals who are out of medical care in Berrien County into care. About CARES: The mission of CARES is to maximize the quality of life for those affected with HIV/AIDS and to minimize the transmission of the disease. www.caresswm.org Care Coordinator- The purpose of the Care Coordinator is to link and retain out of care HIV positive clients into medical care. These activities will be focused towards initiating; linking and sustaining optimal HIV medical care for clients. This position will work alongside the Lakeland and Intercare HIV care team to identify clients who are out of care, connect with those clients, assess their needs and refer to appropriate internal and external services. Integrates HOPWA and MDCH prevention and perinatal prevention objectives into client service plans by regularly addressing housing and prevention needs with each client. Additional responsibilities include, consultation with clinical and hospital staff to protect the privacy of and advocate for health of pregnant women living with HIV, assist MDCH and MATEC with the coordination of perinatal HIV training for medical and support staff throughout the state, provide support to perinatal treatment. Bachelor's degree in nursing, social work, health or related field and one year of case management experience; OR, any combination of education, training and experience which demonstrates the ability to perform the duties of the position. Peer Patient Navigator-The peer patient navigator will work with identified clients who have been lost to care and provide care coordination through necessary healthcare and other systems, including obgyns, hospitals and birthing centers. This will include transportation, health literacy education, assisting through various systems as well as relating as a peer. Team members contribute to the quality assurance process via client case review each week, supervised by the Director of Client Services. Program Consultant (short-term contract)- This independent contract position researches the larger issue of why clients are out of HIV care in Benton Harbor and why that percentage is the largest in the state. The Program Consultant will conduct focus groups made up of clients who have been out of care to find out their barriers to care and provide a report and data back to the Director of Client Services. This position will also be expected to interview medical care professionals, health department personnel, clients and other professionals that interact with possible clients. This position will also create an evaluation process for measuring our Benton Harbor linkage to care efforts. The program consultant will also create a client advisory board from this area and set the perimeters and expectations for the group. Experience in research and program evaluation required. Early Intervention Specialist (short-term contract)- The purpose of the Early Intervention Specialist is to do intensive outreach with the purpose of linking out of care HIV positive clients into medical care. These activities will be focused towards initiating; linking and sustaining optimal HIV medical care for clients. Bachelor's degree in nursing, social work, health or related field and one year of case management experience; OR, any combination of education, training and experience which demonstrates the ability to perform the duties of the position. Ability to: • Assess the needs of low-income people and identify appropriate solutions; • Demonstrate tact, initiative and sound judgment under challenging circumstances; • Work well with people of diverse social, cultural and economic backgrounds; • Maintain strict confidentiality; • Network with community human resource agencies; • Determine treatment plans based on client assessment; • Initiate appropriate treatment or referrals; • Prepare complete and accurate reports and records; • Establish and maintain effective working relationships with clients, employees, other agencies and the public; • Follow written and verbal instructions; • Communicate effectively verbally and in writing.
