and payment must be postmarked no later than Friday, September 18, 2015.

Vendors will receive an email once their application has been approved.
Vendor space numbers will be sent via email and will also be available for pick-up
in the CRC office on October 2, 2015.
Space Size: 12’ x 12’. Be sure to indicate if you are reserving more than one space. Set-up is from 7:30am-9:30am. Tear down will begin at
5:15pm. Vendors are responsible for removing all garbage created at their booth. Tables and chairs will not be provided, please remember
to bring your own. You must supply your own tent/canopy. Bricks are recommended.
I/We the undersigned participants/vendors, listed below, specifically assume any risk and release the Clearfield Revitalization Corporation
from any and all causes of action, claims, demands, liability, or damages, and I/we agree to indemnify and hold the Clearfield Revitalization
Corporation harmless from any and all costs of any kind whatsoever (including attorney's fees and court costs), cause of action, claims,
demands, liability or damages relating in any way to the Clearfield Revitalization Corporation's functions and or events, whether caused by,
or allegedly caused by any acts, intentional, negligent, contributory, or otherwise by the Clearfield Revitalization Corporation agent,
officers, directors, employees. I/We understand the Clearfield Revitalization Corporation is not responsible for any accidents, injuries, or
lost articles during the festival. No refunds, no exceptions. Undersigned is responsible for payment and collection of all taxes. No subleasing or subletting spaces to other parties. I/We agree to abide by the rules set forth in this application and understand that a failure to
abide by these rules may result in our expulsion from the festival. By signing below and by participating in the CRC Fall Festival, I/we give
the CRC the right to photograph my/our booth, products, employees, volunteers or customers and to use these photographs for
promotional purposes.
