S. Orange Ave Orlando, Florida 32801 Monday thru Friday between the hours of 8 A.M. to 5 P.M. excluding holidays.
You have 60 days to pay the $158.00 penalty. After the due date has expired, a Uniform Traffic Citation will be
issued as per FL statute 316.0083 in the amount of $262.00.

2. PAYMENT BY MAIL: use the enclosed red envelope with the top portion of this Notice of Violation. PLEASE DO NOT
MAIL CASH. Be sure to put the Notice # on your check or money order.
3. PAYMENT BY PHONE: call 1-855-536-8259. You will need your Notice # and PIN printed on the front of this Notice.
4. PAYMENT IN PERSON: with a check, cash, money order, or credit card (Discover, Visa, or MasterCard) at the City Hall
Building – City of Orlando, located at 400 South Orange Ave., First Floor Orlando STOPS Cashier; Monday through Friday
between 8:00am and 5:00pm.
5. SUBMISSION OF AFFIDAVIT OF DEFENSE: As the registered owner of the vehicle, you are deemed responsible for the
violation and the payment of a $158.00 penalty unless, by the due date, the owner can establish that pursuant to
F.S.316.0083 Florida Statutes, the motor vehicle:
(a) Passed through the intersection in order to yield the right-of-way to an emergency vehicle or as part of funeral
procession;
(b) Passed through the intersection at the direction of a law enforcement officer;
(c) Was, at the time of the violation, in the care, custody, or control of another person;
ACCEPTANCE OF THIS ASSERTION BY CITY OF ORLANDO WILL RESULT IN THE ISSUANCE OF A NEW NOTICE OF VIOLATION
(NOV) TO THE PERSON NAMED ON THE AFFIDAVIT IN THE AMOUNT OF $158.00.
(d) A Uniform Traffic Citation (UTC) was issued by a law enforcement officer to the driver of the motor vehicle for the
violation of s. 316.074 (1) and s. 316.075 (1) (c) 1, Florida Statutes, as stated in this Notice.
(e) Vehicle which is part of a Funeral Procession being lead by a funeral escort vehicles; funeral lead vehicle.
(f) The vehicle’s owner was deceased on or before the date of the violation.
6. The affidavit must contain detailed information supporting an exemption, and be sworn before a notary public or
another person authorized to administer oaths.
7. If your affidavit complies with the requirements of s. 316.0083, Florida Statutes, no further action will be taken
against you, and you will receive a notice of dismissal.

The submission of a false affidavit is a misdemeanor of the second degree, punishable under s. 775.082 or s. 775.083,
Florida Statutes, by a term of imprisonment not to exceed sixty (60) days and/or fine not to exceed $500.00.

9. No points are assessed against your driver’s license in the State of Florida for a red light
camera violation. Also, payment of this violation will result in no record entry linked to your
driver’s license. However, the issuance of a Uniform Traffic Citation may result in a record
entry linked to your driver’s license.
