4 Have your TV purchase sales receipt handy.
4 Visit www.mySEARSrebate.com to create a free account and
submit your purchase information. Be sure to use the same
phone number associated with your DISH Network account.
4 You will receive an e-mail every step of the way: 1) after your request is received, 2) when your qualification is verified and in process and 3) when your Prepaid Card is mailed.
After your DISH Network service has been installed and you’ve
submitted your purchase information online, allow 6 – 8 weeks for
your Prepaid Card to arrive.

check the status of your Prepaid Card.
