PLAYER REPLACEMENT & ADDITIONS FORM
General Information
Use this form to replace one or more players on a Casper 3 on 3 Tournament team OR to add a fourth player to a team
of three players. Once Casper 3 on 3 Tournament receives your entry form, any team roster changes or additions must
be requested on a Player Replacement/Additions Form and will only take effect once approved by the Casper 3 on 3
Tournament Staff. Teams having players that are not properly registered for the tournament will be disqualified. NO
ROSTER CHANGES ARE ALLOWED AFTER YOUR FIRST SCHEDULED GAME!!
Who Qualifies?
It is the discretion of the tournament staff to approve any player changes/additions since the new or additional player(s) cannot alter
the team’s placement in the tournament. A replacement player must fit in the same age category of the deleted player.
Deadlines / Fees
All player replacements and additions must be submitted at the team check-in on Friday evening of the event weekend or prior to your
team’s first scheduled game.

Every player and their parent/guardian, if the player is under 18, must read and sign this Form. By signing, each person signifies that he/she has read, fully understands and
voluntarily agrees to abide by this information. There are risks associated with participation in this tournament and its related activities, and the undersigned voluntarily and
knowingly accept and assume such risks. In consideration for allowing Participant to participate in the tournament, and on behalf of myself, my heirs, executors, administrators
and assigns, I hereby waive, release and discharge World Events Sports Properties and partners, all event sponsors, event charities and any of the agents, employees, officers and
directors of the foregoing from any and all claims for damages, actions or suits and demands whatsoever in law and in equity, including but not limited to, the risk of injury from
playing in this tournament and the risk of loss of personal property by theft or otherwise. The undersigned also hereby grant full permission for event organizers, including the
World Events and partners, to record any or all of Participant’s participation in this event for photos, motion pictures, TV, radio, recordings, videotapes and other media known
or unknown, and to use them no matter by whom taken, in any manner for publicity, promotions, advertising, trade or commercial purposes, without any remuneration of any
kind due me.
