Notary Publics and Authorized Representatives/Agents (pg. 5)
If the department hires an employee who will be paid by the university yet will work at a distant location
(not physically at the university) the I-9 Form still must be completed if the employee resides and works
within the U.S. The department can choose an authorized representative /agent to complete the I-9
Form for the University.

An authorized agent can be anyone that the departmental personnel assign to act as an agent
for the UC (outside personnel officer, attorney, notary public, etc.)
The representative/agent should be chosen carefully, since the university will be held
responsible for the actions of the agent.
If the I-9 Form is completed by an authorized agent, the agent must enter the university’s name and
address in the Certification section. In order to accommodate this change the Remote Hire Notary Form
has been updated to include:

A change in title from Remote Hire Notary Notice to Remote Hire Authorized Agent Notice Form
Updated form instructions to notify the agent to use the university’s information
Added fields to indicate campus information.
When completing the Certification section of the form the agent should do the following:
1. The notary public or authorized agent signs the form.
2. The agent prints his/her full name and title.
3. The agent indicates the university’s name and address. This information will be provided by the
department representative on the Remote Hire Authorized Agent Notice Form.
4. The agent indicates the date he/she reviewed the required documents.
