Where a young person is in Year 9 or above, this plan must include identification of needs, outcomes and provision required to assist in the
preparation for adulthood, including: further education; training and employment; independent living; participation in society and being as
healthy as possible in adult life.
The lead professional [e.g. SENCO, class teacher or tutor], together with the appropriate specialists, should consider a range of well-founded and effective
teaching approaches, appropriate equipment, strategies and interventions in order to support the child/young person’s progress and achieve these
outcomes.

The arrangements the education/training provider has in place for setting and reviewing shorter term goals: Agencies (including schools) will
maintain a plan that sits alongside this EHCP, which allows for shorter term targets to be set and which can be reviewed regularly to ensure the
child/young person is on track to achieve the outcomes specified below.
This plan must be reviewed at least annually. The first review must be held within 12 months of the date of the issue of the EH&C Plan.
