1. All sections of the application form, including APPENDICES I, II, III and IV are to be duly
completed and signed before submission. Incomplete submission will be rejected.
2. In APPENDIX II, please provide the details of your past employment/ practical experience (up 10
Projects in the past 12 months)
3. RE/RTO is required to attain the minimum * 8 STU on a yearly basis for renewal.

6. Starting 14th September 2015, APPENDIX IV, Performance Assessment Report for RE & RTO
(C&S) by QPS are compulsory for each project supervised during the 1 year period before each
renewal
7. Multiple forms (APPENDIX IV) are required if the RE/RTO had supervised more than 1 project
during the ONE year period before each renewal
8. Testimonial from present Employer/Professional Engineer is to be submitted with this application.
9. RE/RTO above the age of 65 is to produce a medical certificate certifying that they are physically
fit to work on construction site for the renewal of certification; to be submitted together with
renewal form.
10. Professional Engineer (PE) with a valid practicing certificate in Civil & Structural discipline is not
required to register as RE/RTO.
