Which Option Suits you best?
OPTION 1: - SALARY DEDUCTION ARRANGED DIRECTLY BY NHS:Annual membership £285.50
Receive 13 months instead of 12
Payments are spread evenly across 13 pays at £21.96 per month
How it works:NHSGGC pay the annual membership fee to the Glasgow Club. Staff sign a salary mandate
form and the payments are deducted from your salary for 13 months at £21.96 per month.
To join the scheme: Simply choose the month you want your membership to start and submit your application
to the address below at least 2 weeks in advance. Please note, you are agreeing to a full

PLEASE AVOID CONTACTING ALEX BY TELEPHONE. This guidance should provide you with
all the information you need to complete your application.
3. If you are currently a Glasgow Club member, your direct debit payment will be cancelled
and you will receive written confirmation from the Glasgow Club. We recommend you
cancel the direct debit mandate at your bank and also contact the Glasgow Club Customer
Service team on 08000 270 470 or 0141-287-8931 to confirm. Alternatively, email them on
[email protected]
4. Your memberships will be effective from the first day of the month you apply to start,
provided 2 weeks notice is given. To activate your membership simply drop into your local
leisure centre with your pay number and collect your new membership card.
5. Once your membership has been activated, check £21.96 is being deducted from your
salary each month; there may be a delay of one month before this is initiated by payroll.
6. This membership is on a rolling basis and will carry on year on year. If you do not wish
to continue your membership next year you should inform Alex Dawson in writing at least
one month before the 13th month of your membership.

(A minimum 12 month contract which runs until notice of cancellation is given)

(one month minimum term with up to 30days notice to cancel)

(12months initial membership with a free month if you renewed before expiry)
How it works:Sign up directly with the Glasgow Club and choose to pay by monthly direct debit on the
7th of each month or an annual lump sum.
To join:Simply drop into your local venue with up to date proof of address, a wage slip and bank
details if you have chosen to pay by direct debit and the Glasgow Club will get you started
straight away!
