The parent or guardian signing the following permission slip
will be contacted in the event of an emergency. I, the parent
or guardian of the participant above hereby authorize the
agents, officers, and employees of the United 1996 Soccer
Academy camps to act for me according to their best
judgment in any emergency requiring medical attention, and
hereby waive and release agents, officers, and employees of
the United 1996 Soccer Academy camps, owners and or of
facilities, from any and all liability, injuries, illness or loss of
property incurred while attending camp. Parents/guardians
must inform the camp director of any special health needs.
United 1996 Soccer Academy reserves rights to cancel due to
lack of campers.

Jan Franson
Girls Coaching Director
Jan has been coaching with United 1996 F.C. for
eight years and currently holds the USSF "D"
license. Born in Norway , he grew up playing
the sport and advanced to play on his regional team from the
ages of 13-16. Since coming to United 1996 F.C., he has
head-coached and assisted in coaching both boys and girls
ranging in age from 10-16 years. He is currently part of the
Kentucky ODPG coaching staff.
Craig Jones
Craig is in his sixth season as Head Men’s
Soccer Coach at Hanover College where he
has amassed school records for single season
and all-time wins. He played collegiate soccer
at Western Washington University where he
led the Vikings in scoring and was an All-Conference standout
his junior and senior seasons. Jones came to Louisville to
pursue his Masters in Sport Administration from U of L and has
coached competitive soccer in the area for the past 10 years.
He was the top assistant at Bellarmine University for two
seasons before taking over the head coaching duties at
Hanover. Coach Jones holds his USSF “B” license and heads
the 92 Internationals for United 1996 F.C. Craig was also voted
“Coach of the year” in the Heartland Collegiate Athletic
Conference in 2006.
David Gross
This is David's third season coaching for
United 1996 F.C. A long-time player with the '86
Boys, David is the first United 1996 F.C. player
to return to the club as a coach. David holds the
USSF National Youth Coaching License and his USSF "D"
level Coaching License.
Anatoly Berezov
Anatoly Berezov's extensive experience
includes professional and youth soccer clubs in
Moscow, including professional team Nika
Futbol Klub, and youth clubs Trudovie Rexervi,
Central Club F.S.H.M., and Chertoanovo. He
has coached many players who have gone on to the Russian
National Team and Junior National Team, as well as
professional club Teams in Germany, Portugal, and Russia.
Coach Berezov holds a Soccer Coaching degree from the
State Central University of Physical Culture in Moscow,
Russia.

Muhamed Fazlagic
General Manager
Muhamed came to this country in
1995 and settled in Louisville to be
closer to members of his family.
Muhamed founded United 1996 F.C.
in the summer of 1996, serving as director of
coaching, and quickly brought in an outstanding
coaching staff.
Muhamed holds the USSF "A" license and the
NSCAA Advanced National Diploma. He also holds
the Royal Dutch Soccer Fedederation Advanced
National Diploma. He has been on the coaching
staff of the Region II ODP (Olympic Development
Program) since 2004. Muhamed has completed
first part of theUnited European Football
Association ‘A’ Coaching License School .in
Great Britain.
Muhamed holds a Master of International Administration and Development degree, and is the
associate head men’s soccer coach at
Bellarmine University.
Thabane Sutu
Goalkeeper Coach
Thabane has been the goalkeeper
coach for United 1996 F.C. since
2001. He anticipates getting his
NSCAA Advanced National Goalkeeping Diploma
this summer. His professional experience
includes Arsenal Football Club in Lesotho,
Southern Africa, and the National Sporting Club,
"Al-Ahly" Egypt. He represented Lesotho in U16,
U20, U23 and captained the Senior National Team
from 1994 until 1997. Major Honors include the
African Cup Winners Cup with Al-Ahly (as a
backup goalkeeper) in 1993 against Africa Stars of
Ivory Coast, winning the Arab League Tournament
with Al- Ahly in 1995, and League Champions with
Al-Ahly from 1994-1996.
Girls & Boys
Pre-Competitive Camp
The emphasis for this camp is on learning through
fun. This camp is offered to the young soccer player
with the goal of playing Select Soccer, or to improve
his/her basic skills of the games.
Correct ball handling techniques are introduced along
with the start of the basics of team play and positions.
The girls/boys will scrimmage against each other on
small-sided fields. The goal is to give all players as
many touches as possible on the ball while being
instructed on the correct methods for the development
of good soccer techniques
4 v 4 Dutch Soccer System
By putting players into small-sided games,
they will develop the appropriate foot skills,
field vision, understand of effective positioning,
and learn the possession style of soccer for
which United 1996 Futbol Club teams are
recognized.
Half-Day Option
Boys and Girls ages 5-8 may choose a half-day
option, 9am to 12pm, for any of the camps. The
half-day option is $85.00.
Camp Equipment
These camps are offered to the soccer player striving to
play middle school and high school soccer. These
camps will focus on the improvement of the technical
as well as the tactical aspects of the game.
Each camper will receive a complimentary Nike
T-shirt. All campers are required to bring proper
playing equipment including water, cleats, shin
guards, extra socks, ball, sun screen, etc.
Campers should also bring two T-shirts: One dark
and one white or light colored. Those campers
that will be attending all day camps should also
bring snacks and lunch.
This camp will place middle school age soccer players
in an environment that prepares them for what is
required to play at the next level.

Middle School Soccer Camp
This camp will advance the camper’s knowledge of the
more detailed aspects of the game through several
means including coaching sessions, discussion, college
and professional demonstrations. Campers will
scrimmage in both small-sided games and full field
competitions.
Goalkeeper Camp
Goalkeeper Camp is designed to help young athletes to
gain the skills necessary to compete at higher levels,
primarily Select Soccer and High School. The camp will
be conducted by Thabane Sutu, former Lesotho
National Team Goalkeeper. Thabane is a well-known
master of Goalkeeping in the greater Louisville area.
Potential campers will have a lot of fun and will learn
even more!
