2014 NOMINATION REQUIREMENTS
BOSTON MARATHON® JIMMY FUND WALK
Please write, in 500 words or less, why the team captain you have nominated deserves to be recognized for his or
her Walk efforts. This description should include specifics as to how he or she:

Stands out among team captains

Inspires teammates

Is a dedicated recruiter and fundraiser

Demonstrates an unwavering commitment to help Dana-Farber conquer cancer
Please note that the award is being judged solely on the participant’s involvement in the Boston Marathon Jimmy
Fund Walk and not other Jimmy Fund or Dana-Farber activities.
QUALIFYING CRITERIA: The walker must raise the fundraising minimum of $300 by October 15, 2014 and
must be a captain or co-captain of a 2014 Walk team of five or more walkers.
NOMINATING PROCESS: Send nomination in writing to [email protected] with the subject
line: “Team Captain of the Year: [Name of nominee]”, by October 31, 2014. Only nominations which include
descriptions, rather than a sole subject line with the nominee’s name, will be considered. Thank you.
