REGISTRATION CHECKLIST
Team registration for the Ohio Galaxies Showcase will be by by ONLINE submission ONLY. Be sure to
confirm your method of registration (US Club Soccer or US Youth Soccer) and follow the requirements for
team registration listed below. Please be sure that you include all the necessary forms which can be found on
our website.
We will not accept forms via email, fax or mail.
US CLUB SOCCER TEAMS:
Certified / Validated Original 2016-2017 US Club Team Roster
Copies of validated US Club Soccer Player Pass for each player (including guests)
Certified / Validated US Club Soccer loan / guest player forms (if applicable). All loan / guest players
must have a validated / laminated 2016-2017 US Club Soccer Player Pass
Team Contact Form
OGFC Liability Release Form signed by ALL parents / guardians for each player
AIA Liability Release Forms signed by ALL parents / guardians for each player
(Only required if your team plays a match at AIA)
Copy of Concussion Training Certification for ALL carded coaches

USYSA TEAMS:
Certified / Validated Original 2016-2017 USYSA Team Roster
Copies of validated USYSA Player Pass for each player (including guests)
Permission to Travel (teams that reside outside of Ohio South only)
Guest Player Roster – Approved / validated by the State Association where player is registered
Team Contact Form
OGFC Liability Release Form signed by ALL parents / guardians for each player
AIA Liability Release Forms signed by ALL parents / guardians for each player
(Only required if your team plays a match at AIA or University of Dayton)
Copy of Concussion Training Certification for ALL carded coaches
