Work and school 1. Comply with both conditions below 2. Comply with either of the two conditions below a. Attend an educational or vocational program provided by a local school board, or any place of learning, training or recreation approved of by the youth worker and provide proof as required by the youth worker and sign releases to allow the youth worker to confirm your attendance. or b. Make reasonable efforts to seek and maintain suitable work approved of by the youth worker and provide proof as required by the youth worker 3. ________________________________________________________________ 9. Weapons and Permits 1. Do not possess any weapon(s) as defined by the Criminal Code (for example, but not restricted to: a BB gun, pellet gun, firearm, imitation firearm, cross-bow, prohibited or restricted weapon or device, ammunition or explosive substance or anything designed to be used or intended for use to cause death or injury or to threaten or intimidate any person) 2. If you now possess any of the items referred to in the above condition you must surrender them to the police within: _____ hours or if in custody, within ____ hours of your release from custody 3. All licences and registration certificates pertaining to these things referred to above, shall be surrendered as follows: _______________________ and do not apply for any such licences and registration certificates. 4. Alcohol, Drugs 1. Do not buy, possess or consume alcohol or other intoxicating substance 2. Do not go into any liquor licenced premises 3. Do not possess or consume any unlawful drugs or substances (refer to the Controlled Drugs and Substances Act) except with a valid prescription in your name or those available over the counter 4. Do not possess any weigh scales, hydroponic growing equipment or other drug paraphernalia 5. Do not be in the driver’s seat of any motor vehicle unless your blood alcohol concentration is zero 6. _____________________________________________________________ 11. Counselling and Treatment 1. Attend and actively participate in all assessment, counselling or rehabilitative programs as directed by the youth worker and complete them to the satisfaction of the youth worker for: anger management substance abuse alcohol abuse domestic violence parenting psychiatric or psychological issues stress management bereavement/grief issues literacy financial counselling life skills __________________________________________________ November 2015 Page 9 2. You shall sign any release of information forms as will enable your youth worker to monitor your attendance and completion of any assessments, counselling or rehabilitative programs as directed 3. You shall provide proof of your attendance and completion of any assessments, counselling or programs as directed. 4. ________________________________________________________________ 12. Community Service 1. Perform ____ hours (not to exceed 240 hours) of community service work on a rate and schedule to be directed by the youth worker but must be completed within ____ months of the start date to this Order (not to exceed 12 months) 2. Perform ____hours (not to exceed 240 hours) of community service work. This work is to start no later than _________ and shall be completed at a rate of not less than _____ hours per month. You shall complete the work as directed by and to the satisfaction of the youth worker. You must complete all of your community service hours by/within _____ months (not to exceed 12 months) 3. These hours are in addition to any community service hours that may be required of you by school.(Reference the "Ontario Secondary School Diploma Requirement: Community Involvement Activities in English-Language Schools" in the Education Act) 4. Credit or Debit Cards, I.D. and Data 1. Do not possess any identification, card with a data strip or security chip, credit or debit card, credit or debit card data, a blank card with a magnetic strip, cheque, negotiable instrument or banking document unless it has been lawfully issued in your name EXCEPT: a. such items in the name of: ________________________________ b. purposes of employment, namely __________________________ c. ______________________________________________________ 2. ___________________________________________________________ 15. Restitution 1.
