Screen Use: The IPMD screen is

Scenario: As a certifier, you have received the provider
application and a provider number has been assigned through the
IPMD screen; now you must open a new certification for the
provider.
This section of the IIS screens training will
teach you how to access the IPMD screen
to open a new certification.

Open a blank DHR screen,
Type IPMD,provider
number,fb,new and then
press Enter.
The Foster Care Certification Update
screen is displayed.

Press the Tab key to the
Cert Dates: Begin field,
Type the date.
The date entered in this field should be the
date Central Office approves the Consent
for Criminal Records & Fingerprint Check
(1011F).

Cert Dates: End field, Type
the date.
The end date is one day
prior to the year end.

The curser will move to
Cert Term field, Type ‘a’ for
annual or ‘p’ for provisional.

The curser will move to the
Capacity field, Type the
maximum number of
children to be in the home.

Press the Tab key to the
Pher Sex field, Type the
preferred sex of children to
be in the home.
The Pref Sex code options are: M for male,
F for female and E for either.

Pref Age: Min field, Type the
minimum age of children
allowed in the home.

Pref Age: Max field, Type the
maximum age of children
allowed in the home.

The curser will move to the
Contract Present field, Type
‘y’ for yes.

to the Cert Usage
field, Type ‘r’.
Certification Usage codes:
P = Foster Home Certificate (private agency use)
R = Regular Foster Home Certificate
S = Special Foster Home Certificate
A = Foster Home Certificate (adoptive use)
O = Other Foster Home Certificate (Indian home)
Data entry for the Schools field is not
mandatory. However, you may choose to list
the schools the child may attend while in
care at the foster home.
See individual division school code sheet.
Each school within a division area are
assigned a 3 digit code.
Your next data entry will be in the Cert Action
field. You must choose the appropriate code
for this field. The next slide will display a list
of Cert Action field codes and the definition of
each code. You may also find the codes
listed on form CF 332F.

fields is not mandatory.
If the provider has requested a payee
designation, you must type ‘Y’ for yes in
the Need To Update Check Or Payee
Designations field.
If the provider has requested a specific place for
where the check is to be mailed, you must type a
code in the Check Designation Field. The codes
are:
FA Foster Home Address
FM Foster Home Mailing Address
If the provider has requested that the check be
payable to a specific person, you must type a
code in the Payee Designation Field. The codes
are:
N Provider Name 1
C Provider Name 2
J Joint
Press Enter
to save.
When all data entries have been completed in
the IPMD screen, press the Pause Break key.
A blank DHR screen will appear so that you
can complete work for other screens.
