If there is different information for your vessels please indicate to which vessel information applies (sister ships can be filled out in one questionnaire). A. Ships particulars (please provide a copy of the list of particulars in addition to the following): Seagoing Ballast Draught Total Water Ballast Capacity (identify unit of measure) List all ballast water tanks & capacities (indicate location by frame #) Identify the appointed ballast water management officer (designation / rank of officer) Provide any other relevant information B. Ballast Water Tank Arrangements & Capacities Identify the pumps available to be used by each Water Ballast Tank also identify the capacity of each of those pumps (ie. M3 / hour). For a class approved plan, provide a copy of the tank arrangement plan. * Please provide a full sized pumping plan * Please provide a full sized capacity plan ECM Form 06 Version 1-2015 1 of 7 ECM GD – 06 – ECM BALLAST WATER MANAGEMENT PLAN QUESTIONNAIRE C. Ballast Arrangements for the Following Conditions For a class approved plan, please include the ballast calculations from your computer for each condition (this is required). Condition: Ballast Calculations: Minimum departure ballast in fair weather: Normal ballast condition: Storm ballast Any other relevant conditions: D. Conditions in which ballast water exchange is not to be undertaken Conditions: E. Procedures for managing ballast water Are there procedures in place for managing ballast water? If so please provide (such as from the SMS manual). F. Procedures for informing shore management when ballast exchange starts Are there procedures that are in place in which the vessel informs the shore management when ballast exchange starts and ends? When ballasting is not possible? If so, please provide. ECM Form 06 Version 1-2015 2 of 7 ECM GD – 06 – ECM BALLAST WATER MANAGEMENT PLAN QUESTIONNAIRE G. Sediment removal or reduction Are there any reasons why the flushing of ballast tanks would not be feasible for the vessel? If so, please provide. Please explain how the ship normally cleans ballast sediment (such as by contract labor, ships crew, during each drydock, etc.) H. Retention of ballast onboard Does the vessel undertake ballast operations for controlling list & trim during cargo operations? If so, please provide plan for internal ballast water control to minimize the discharge of water. I. Water treatment methods Does the vessel use any water treatment method to treat ballast water, i.e., heat treatment, exposure of ballast water to ultra violet light, filtering and / or chemicals? If so, please provide details. J. Exchange at sea Identify the method of at sea ballast exchange; Sequential method or Flow-Through method. Sequential method: in which ballast tanks are pumped out and then refilled with water. Flow through method: in which ballast tanks are overflowed by pumping in additional water to dilute the original water. ECM Form 06 Version 1-2015 3 of 7 ECM GD – 06 – ECM BALLAST WATER MANAGEMENT PLAN QUESTIONNAIRE K. Order of ballast exchange Are there procedures in place which address, step by step, tank by tank showing the order of exchanging ballast? (either sequential or flow through) If so, please provide (this is required for class approved plans). Note: the procedures should identify the maximum number of ballast tanks to be worked at one time and express stress limits and the draught at the end of each step. Note: if the vessel can use either the sequential method or flow through method, procedures for both methods must be provided. L. Ballast sampling points Are there procedures in place for sampling ballast water tanks? If so, please provide. Identify the sampling points for all tanks/spaces used for ballast water. For class approved plans, you must provide a diagram or drawing of the sampling points. Are there procedures in place for taking samples from the ballast pipeline? If so, please provide. Identify the sampling points for pipeline sampling. Example: sample to be taken from the drain cock on the emergency fire pump in forward stores, etc.) ECM Form 06 Version 1-2015 4 of 7 ECM GD – 06 – ECM BALLAST WATER MANAGEMENT PLAN QUESTIONNAIRE M. Sediment samples Identify the location of access to all ballast tanks for sampling sediment. Are there procedures for disposal of sediment at sea? N. Crew training & familiarization Specify all onboard/shoreside training the vessel crew undergoes with respect to ballast operations and related regulatory requirements (included in familiarization, on-board videos, addressed in hand-over checklists, etc.). O. Duties of the appointed ballast water management officer Are there any other written duties applicable to the ballast water management officer with regard to ballast operations and company policy? If so, please provide. Please provide any other information you feel is relevant to above listed questions. Ballast Water Treatment System Supplement In addition to the information required on Form 6, would you please address the following concerning the installed BWTS equipment. Also please include an electronic copy of the operations manual for the system as that is required for class review. 1. Treatment System Name Technology (Chemical treatment, Filtration, etc.) Operation (Ballasting, Deballasting, Circulation, other) Capacity Location 2. Oilrec Does the ship have an OILREC notation? 3. Yes No Treatment equipment details Name of the manufacturer Type and model of the equipment installed Type Approval Certificate no List of the major components of the treatment system (PLC panels, electrical cabinets etc) : Item 4.
