RE: PERSONAL ACCIDENT POLICY NO: IAS 82827
Team members covered while playing, training, travelling to or from football matches other than by air.
The above is Medical Expenses only Policy with limited Capital Benefits.
Capital Benefits:
Death
Loss of eye or limb, permanent total disablement €75,000
Limit any one Claim €2,500 for Medical Expenses
Limit of physiotherapy treatment is €400 per person, per year
Dental - otherwise unrecoverable expenses limit €1,000
EXCLUSIONS:
The first €100 of each claim is excluded.
CLAIM FORM MUST BE COMPLETED WITHIN 30 DAYS OF INCIDENT OCCURRING
SUBSEQUENT MEDICAL RECEIPTS TO BE SENT TO FLANNELLY INSURANCES

Notes:
Any Club with Changing Facilities must write to us with details so that Liability
Insurance can be effected on same.
Public Liability Policy does not cover spectator stands or clubhouses. Details of same
