5.6.3 Waiving of Charges
Course Fee Reduction or Waiver Application Form
The decision to waive or reduce tuition fees for Applied Education students rests with the Director –
Brett Thornett, Applied Education, Level 1, 524 Hay Street, Perth, WA 6000. This decision will be in
line with the Vet Fees and Policy Charges 2014 – 5.6.3 Waiving of Charges.
Fee waivers may be considered on the basis of exceptional circumstances that have led to financial
hardship or on the basis of other extenuating circumstances.
In submitting this form, you consent to the Director of Applied Education collecting, using and
disclosing the personal information of you for the purposes of:
• assessing your application for fee waiver, including reporting and exchanging information
generally with Traineeship Manager, Applied Education, any other relevant Western
Australia entity or person that you refer to as evidence to support your claim.
• liaising with, providing information to and receiving information from the Australian
Department of Immigration and Citizenship in relation to your compliance with all applicable
migration laws and visa regulations and conditions if necessary.

Section A: Evidence That Supports Your Application For A Fee Waiver
Assessment of Financial Hardship
In making your case for financial hardship it is helpful if you provide details and evidence which
shows:
• ALL sources on income including pensions and/or allowances paid to you and/or family
members; and
• provide other details and documentary evidence that supports your claim of financial
hardship.
In your application, please tell us if you receive any of the following:
• pensions or allowances paid by Centrelink
• pensions or allowances under a reciprocal agreement between your home country and the
Australian government
• a pension or benefit from another federal agency
You can also tell us if you, or immediate family member or sponsor have claimed/sought assistance
from an agency but were turned down.
It will help us if you provide evidence to support your claim. Some possible documents might be:
• a Centrelink Income Statement or letter outlining your Centrelink pension(s) and/or
allowance(s)
• a letter from another government agency outlining your pension(s) or allowance
• A letter from a non-government organisation detailing your financial circumstances.
If you and/or family members do not receive any pensions and/or allowances then in your
application, please tell us the details that support your claim of financial hardship and provide
relevant documentary evidence.
Exceptional Circumstances
Please give details of you and your family’s specific circumstances. We will use this information to
determine if, in addition to financial hardship your situation also qualifies as exceptional
circumstances
Health and disability
Have you, a family member and/or sponsor experienced a sudden and/or unexpected health issue or
disability? Your circumstances may have:
• restricted your income;
• increased the amount of money you are spending; and/or,

• increased the amount of care you need to provide for a family member.
It will help us if you provide evidence to support your claim. Some possible documents might
be:
• A letter from doctor/hospital or medical specialist
• Medical expenses and/or bills
• A letter from a specialist support agency (e.g. Stroke association; Leukaemia Foundation)
Have you experienced the death of a significant wage earner?
• A death certificate
Family responsibility
Is there a new situation for you and/or your family?
Here are some examples of circumstances which may cause financial hardship:
• Care of an orphan /additional family member
• Relationship breakdown
• Family violence.
It will help us if you provide evidence to support your claim. Some possible documents might be:
• A letter from a support agency or government department; and/or,
• a signed and witnessed affidavit from Australian resident. Please use a witness who has
known you for at least 6 months.
• Domestic Violence Order (DVO)
Additional information: You can provide more information about you and your family on a separate
page.
Other
Sometimes you may be experiencing difficulties we have not listed in the examples above. Please
use a separate page, remembering to include photocopied evidence, if available, to document any
other changes in circumstances you would like to tell us about.

I understand that Applied Education reserves the right to vary or reverse any decision on the basis of
untrue, misleading or incomplete information; and
