he will be eligible for a card.
Provide information about alternate forms of address verification.

Have you always used this last name?
You may need to search more than one last name.
Do you always go by ‘first name’?
You may need to search variations of the first name; middle
names used as first names; and nicknames.
Once you’ve gathered all name information:
Begin your Evergreen Database search.
Don’t be too shy to ask, “Have you always gone by this name?”
Search EI Database For Duplicate Accounts
Here are two patrons with
DoB: 2001-09-17.
One with a middle name
the other a middle initial.
This is a potential
duplicate account.
2007-08-29 is another
potential duplicate: no
middle names listed.
In some cases, patrons
don’t have middle names
and that could be noted
on the patron’s account
for future reference.

Name: Last/Required, First/Required, and Middle/ Optional
Date of Birth/ Required
Primary Identification/ Optional
Parent/Guardian/ Optional
(Driver’s License or ID Card Number)
(Minor’s Account)
Address/ Required: Often unreliable
Optional Registration Fields To Consider
There are very few required fields for
Patron Registration (Highlighted Yellow)
Creating or Updating Patron Accounts
Reasons to consider using Optional Fields
Prefix/Title: Because of gender neutral names.
Middle Name: For better patron searches
OPAC/Staff Client Holds Alias: prints on Hold Slips
instead of the patron name (for privacy); and it is
useful in creating groups (homebound, bookmobile)
Driver’s License: for better patron searches.
Verification, if your library choses to allow patron ID
for Checkout (This option is home library only)
Parent/Guardian: for better juvenile searches. [PSC
is looking at the possibility of moving fees from a
child’s account, on majority, to the parent/guardian
of record. If this happens, this name would be
needed.]
Creating or Updating Patron Accounts
Reasons to reconsider using Optional Fields
User Settings: This group of fields sets the
Default Hold Pickup Location and automated
notification options for the user’s account.
When these fields are used, it is highly unlikely that a
patron would miss a hold notice.
