Dear Parents/Guardians,
Administrative Procedure 5111, Registration and Withdrawal of Students, requires updated proof of residence when
your child enters a different school for any reason. The residency requirement is established by State law (COMAR)
and further clarified by local Board of Education policy and administrative procedures.
A recent view of your child(ren) student record determined than an updated Student Registration Form and proof of
residence has not been provided to school personnel. Please report to the school and provide updated proof of
residence within ten (10) days of receipt of this letter.
Proof of residence may be established by any of the following documents: settlement papers, current tax
assessment bill, deed of property, mortgage statement, current lease rental agreement, or a letter from apartment
complexes on official letterhead. Recent US postal mail from credit card companies, bank, and/or government
agencies should be provided to supplement dated materials. These documents must be originals. Copies of on-line
documents are not acceptable as proof of residence.
If a family is living with a relative or friend, a new updated notarized Prince George’s County Public Schools Shared
Housing Affidavit, signed by both parties, may be used as proof of residence with the following conditions: (1) All
parties must sign the notarized statement; (2) The relative or friend, with whom the parent(s)/guardian(s) reside, must
show their proof of residence; (3) Additional proof of residence (more than one item) is required of the
parent/guardian. Items required to establish proof of residence include, but are not limited to, a statement from an
employer validating the address, recent bank statements, car registration, auto, home, or health insurance policy, or
official government correspondence, delivered by US postal mail. Prince George’s County Public Schools reserves
the right to monitor this type of proof periodically or as needed (as often as every 30 days).

Dear Parents/Guardians,
Please be advised that this is the second notice that you were given ten (10) days to provide our school with updated
proof of residence which is required for transitional students. Our records indicate that you failed to submit the
required documents. Your child(ren) will be withdrawn, will not receive a schedule, and will not be allowed to
attend school for the coming school year if this information is missing. Re-enrollment will require you to
submit the required proof of residence.

Dear Parents/Guardians,
Please be advised that this is the third notice that you have not provided our school with updated proof of residence
which is required for transitional students. Your child(ren) will be withdrawn from school due to failure to
provide adequate proof of residence and will not be allowed to attend school for the coming school year.
Your child(ren) will be re-enrolled when you provide the required updated proof of residence.
Acceptable proof of residence was detailed in our previous communication. I urge you to contact us immediately if
you believe that the requested information was provided or if it can be provided now.
