Q4 Major accident
The movie “Large scale emergency situations”
instructions
In Q4, we have a movie filmed on Maersk Reacher to emphasize large scale emergency situations
and barriers we should have in place to avoid them.
The following is instructions for the facilitator. The movie have 5 breaks, where the facilitator should
stop the movie when the questions are on the screen. We also show the bow tie with barriers – to
highlight the barriers which apply and get the crew to see what bow tie is.
At the end of the movie, there is a strong message from our top management both authorizing,
encouraging and expecting each and every one of us to say stop.
We encourage you to use it on general safety meetings or crew safety meetings, or as a TOFS,
depending on how you find it most suitable for your rig. The movie last for 14 min. + discussions.
We have in the Bow-Tie on the left-hand side technical and procedural barriers as well as competency
requirements. If the barriers -for a reason - should fail we have the right-hand side with pre-defined
actions to take to protect lifes, equipment and environment. BUT – what is the last barrier we have???
YOU!!
The script is thought as a help for the facilitator when running the
Security Film. The theme is Large scale Emergency Situations and
barriers we should have in place to avoid them.
It is recommended for the facilitator to have a look at the HSE-case
with special attention to the “Bow-Tie” section.
Brake 1:
1. Stop the film @ 00:54 min – with the questions on the screen:

Could this happen to us?

Would we proceed with these two positions covered as they are?
1. Ask audience the two questions and open up for a quick
discussion about the topic. Control time!!
You should expect a realistic answer saying “ We are often
in that situation…”

What is happening?

Is he intending to isolate?
Or is he just trying to do a good job?

What is the consequence of the isolation just done?
1. Ask audience the questions above and open up for a quick
discussion about the topic. Spend a little time on the thought
that the Mechanic is new and that he possibly did not intended
to isolate, but just wanted to do a good job - Control time!!
2. Can any in the audience see the consequence of the isolation of
the main & backup air-supply for the unit??

Brake 3:
1. Stop the film @ 06:45 min – with the questions on the screen:

What is happening?

Would we detect a kick if we were losing over the shakers but
the trend stayed stable??
1. Ask audience the questions above and open up for a quick
discussion about the topic. This can be very detailed depending
on audience. Good to see the communication between DFO, SCO
and Logger is well functioning. Could the Driller have a little
thought “The logger is new, what does he know”?? - Control
time!!

What worked here?? – And Suggestions

What did not work?? - And suggestions
1. Repeat the questions above and open up for a quick input from
crew if they see more “What worked” and “What did not work”.
It is important the stress the positive, ie. What actually worked.
Continue – with film off – asking the question:

What thoughts goes through the Driller’s head right now?? Here
there can come MANY strange questions/discussions/comments
etc etc.

from a subsea operation, but on a Jack-Up the spudcans are
standing 10-20 meter from the well we are working on

memory about the right-hand side of the Bow-Tie
