Club: …………………………………………………….. 1 GUIDELINES FOR COMPLETING REGISTRATION FORM AND DOCUMENTS REQUIRED 1. All boxes on the application form must be completed and signed by the parent/guardian and also by a club official. This is usually the Registration Secretary. 2. Whilst a player may have been born in the UK this does not automatically mean the player is British. NATIONALITY I require the 2 documents below to confirm nationality. • A copy of the player’s FULL birth certificate – showing the place of birth of the parent(s). IMPORTANT: The shortened British version only confirms the birth and not nationality. • A copy of the player’s passport. COUNTRY OF BIRTH The player may have been born in the UK but may not be British. If this is the case I need confirmation of their nationality I will require one of the following: • A copy of the player’s British Citizens Certificate. If it has been granted to a parent this must be dated prior to the player’s date of birth. • A copy of the player’s non British passport. LAST CLUB Because of the changes being made to International Transfers by Letter of Approval this is very important. Parents must declare if their child has player before, where and in what country. If this is marked “Note applicable” or “Never played” before I will be required to check with the appropriate country if this is true. If the player has not played before and IS COMPLETELY NEW TO THE SPORT mark with N/A or Never played before. DOCUMENTS REQUIRED TO REGISTER WITH THE ENGLISH ICE HOCKEY ASSOCIATION 1. Application for an International Transfer by Letter of Approval – where applicable.
