• Fast-tracking and district applications available
• Up to $4,000/year
• Schools can apply for funding in multiple years
• Amount requested for PA may not exceed amount requested for HE
• Both HE and PA required for applications
– Choose one of each Play from the 2015-16 Playbook
• Online application format same as last year; schools first select Plays to
generate application
• MMS will continue to send notifications to schools

Starter Form and send to Jessica Harms

Accounting disperses check approximately 10-15 business days
after Application is approved

District notification letter does not appear on Dashboard, must be
sent directly to school via approval letter template
