Gainesville Soccer Alliance 2011-2012 PLAYER/PARENT AGREEMENT Following try-outs, selected players will be offered positions on a competitive team. This agreement must be completed and returned to the club registrar to hold the player’s position on a GSA competitive team. The $250 deposit must be paid for on-line or you can mail a check with your forms. This fee is nonrefundable. In August 2011, all players competing on GSA teams will be carded to FYSA through GSA, provided their club dues have been paid in full (or a payment arrangement has been set). A player remains registered with GSA until the end of State Cup (mid-June) 2012. Any club transfer requires a release of this card from the team and club. The player and parents or legal guardian must be in good standing including the completion of their financial and volunteer responsibilities for a club transfer to be considered. COMPETITIVE TEAMS require the highest level of player skill, and player and parent/legal guardian commitment. Team training begins about August 1, 2011 for all age groups and continues 2-3 times per week until the end of May (and perhaps into June 2012 for State Cup teams) for all players of middle school and elementary school ages. GSA teams with high school age players train until the middle of October (when the high school season starts). They may train intermittently during December (if scheduled to play in a competitive Holiday tournament for the club). Training for the high school teams will resume in February (after completion of high school season) and continue until the end of May (or completion of State Cup). Younger teams without holiday tournaments will break from Mid Nov to Mid January. Competitive teams will have a head coach paid from club fees and may have additional training sessions from the Director of Coaching (DOC) or guest trainers arranged by the DOC. Goalies may also have specific goalie-related training sessions. Costs for player training, field mortgage, soccer equipment, field lining, coaches’ training/licensing, player registration, club computer software/hardware, website expense/maintenance, and club marketing, are included in the basic club fees. The club fees do not include team fees, uniform fees, referees, tournaments and travel fees. The club fees per player will not fully cover the cost of the Director of Coaching, Director of Administration or Jonesville Park Development. These additional expenses will be funded by separate fundraising activities (ie. hosting tournaments, concessions, sponsorships, etc). If a player chooses to stop playing during the season of 2011-2012 for his/her GSA team prior to June 2012 for any reason other than an injury, the parent/player may or may not receive a partial refund for the remainder of the soccer year unless extraordinary circumstances are approved by the GSA Board. The GSA club fees for the 2011 – 2012 are: Total Dues U 9 to U 10 - $500.00 U 11 – U 14 - $700.00 U 15 and above - $600.00 st 1 payment $250.00 $250.00 $250.00 Individual team fees are separate from club fees. nd 2 payment $125.00 $225.00 $175.00 rd 3 payment $125.00 $225.00 $175.00 The schedule of payment for club fees is set forth below according to respective age groups: First Installment: $250.00 due by 6/30/11 as well as the following forms: Player/Parent Agreement, Medical Release, and Photo Release. If new to GSA we will need a copy of your child's birth certificate. Uniform fees: new uniforms are being ordered Second Installment: U 9 - U 10 - $125.00 due by July 30, 2011 U11 – U14 - $250.00 due July 30, 2011 U15 – U19 - $175.00 is due July 30, 2011 Third Installment: U 09 - U 10 - $125.00 due by August 30, 2011 U 11 – U 14 - $250.00 is due August 30, 2011 U 15 – U 19 - $175.00 is due August 30, 2011 Parents with multiple children participating at the competitive level will pay 100% for the first child. A $50.00 reduction will be applied individually for the 2nd child and the 3rd child. To get this discount please contact the registrar so that adjustments can be made prior to payment. Partial club scholarships are available by completing a scholarship form that can be found on the GSA website www.gainesvillesoccer.org under forms. In addition to the above financial responsibilities, all parents or legal guardians of competitive level players will be responsible for a minimum of 12 service hours per year; or, if unable to do so, may donate to GSA @ a rate of $10/hour for the hours not completed (Maximum required donation would be $150 per year and will be tax deductible). All players will be responsible for 6 service hours per year, which may include but is not limited to work at Ronald McDonald House, Top Soccer Program, Refereeing or Coaching in the GSA Recreational Program or Field/Concession work. Players and parents/legal guardians also agree to abide by the Bylaws, policies, regulations and code, of the Gainesville Soccer Alliance, FYSA, USSF, FIFA, and any league, tournament or organization, in which the player represents the Gainesville Soccer Alliance. Players and parents/legal guardian agree to support the decisions of the GSA Director of Coaching, GSA Director of Administration, and the Board of Directors, and will abstain from conduct detrimental to the interests of the Gainesville Soccer Alliance.
