Caistor Yarborough Academy YEAR 9 Application Booklet 2015 – 17 Your pathway to success! NAME: …………………………………………………….. FORM: …………………………………………………….. Please use this page to have a ‘first go’ at selecting your Choices from the list below. Write 1 against your first choice, 2 against your second choice, and so on. For each subject, we need you to complete a Course Choice Form, giving details of why you would like to study that subject. In this way we can ensure that you are making the right choices. We will try to give you your first choices but there are no guarantees. Every choice must be carefully considered and you must give your reasons for the choice. We will use the reasons you give to help decide on your final choices. The Course Choice Forms are at the back of this book. Fill them in when you are sure about your Choices.
