Dear Student, Alumna or Alumnus or Staff member Can you find a few minutes to complete and return the short questionnaire at http://www.surveymonkey.com/s/27XB6BJ to assist us in knowing where and how to continue improving services, premises and equipment for disabled people at LIPA ? If you would prefer to complete a hard copy please e-mail [email protected]. Please note that staff are due to be forwarded a hard copy in due course from Human Resources. You or your responses will not be identifiable in any report produced from all the replies, but where you leave your name and contact details to be re-contacted these details will be forwarded to the relevant person unless you indicate otherwise (at question 5). Also, under the Data Protection Act 1984/1998, we need your ‘explicit permission’ to ‘record, store and use’ data about any disability you declare where you are identifiable, and we will therefore assume that you are giving this permission by adding your name unless you indicate otherwise (at question 5). For the purposes of this questionnaire ‘disabled’ means anyone who considers themselves to have a disability and/or who meets the Disability Discrimination Act definition of people with physical, sensory, communication and intellectual impairments & people with mental health and other long-term health conditions such as diabetes, epilepsy, cancer, multiple sclerosis, HIV and schizophrenia. Impairment must have i) a substantial adverse affect on a person’s ability to carry out normal dayto-day activities ii) lasted for at least 12 months or be likely to last for 12 months or more The term ‘disabled people’ is used in line with our adoption of the social model of disability.
